Osteoarthritis (OA) is a kshayaja-vata-roga or a chronic, degenerative joint condition in which the smooth cartilage covering the ends of bones gradually wears down. As cartilage loses its cushioning and shock-absorbing capacity, the joint is exposed to increased friction and mechanical stress. Over time this leads to joint space narrowing, bony overgrowths (osteophytes), stiffness, and pain that typically worsens with activity and improves with rest. OA is mainly a local, structure-based problemdriven by age, repetitive use, body weight, joint alignment, and muscle strength rather than by immune dysfunction.
How OA differs from Rheumatoid Arthritis (RA) lies in its very nature. OA is a degenerative and mechanically driven disease, whereas RA is a systemic autoimmune inflammatory disease. In RA, the immune system attacks the synovial lining of joints, causing persistent inflammation, morning stiffness lasting more than an hour, symmetrical joint involvement, and often fatigue or other organ involvement. In contrast, OA usually affects specific joints (such as knees, hips, spine, and finger DIP joints), has minimal systemic inflammation, and does not involve immune-mediated joint destruction.
The correct treatment approach for OA focuses on addressing its mechanical and functional causes rather than detoxing as in RA. Core strategies include weight management, strengthening and stabilising muscles around the joint, improving movement patterns, reducing excessive load, and using pain-relief measures when needed. Lifestyle modification and long-term joint care are central, with surgical options reserved for advanced cases. Unlike RA, OA does not require immunosuppressive therapy; instead, successful management comes from
supporting joint structure, function, and adaptability over time. |
